Person/company in US with name, address, phone. For tourism, you can put hotel name and address. [citation:5]
Parents' details, spouse information (if applicable), employment history for last 5 years, education details. [citation:4]
Health, criminal, security, immigration history questions - answer truthfully. Also provide social media handles used in last 5 years. [citation:4]
2x2 inch (5x5 cm) color photo, white background, taken within last 6 months, no glasses. [citation:9]
Review all information, electronically sign, and submit. Print confirmation page with barcode - THIS IS ESSENTIAL. [citation:4]
